भारत में बारह में से ग्यारह गर्भपात ग़ैरक़ानूनी होते हैं, सन् 2001 की जनगणना के अनुसार देश के सबसे शिक्षित और सम्पन्न राज्यों में, प्रति हज़ार पुरुषों के मुक़ाबले स्त्रियों की संख्या में, ख़ासकर 0–5 के आयु वर्ग में, काफ़ी गिरावट आई है; और, भारतीय परम्पराएँ जहाँ प्रजनन और मातृत्व को पवित्र करार देती हैं, वहीं भारत सरकार की नीतियाँ और स्वास्थ्य सेवाओं का ज़ोर प्रजनन को नियंत्रित करने पर है।
ये विरोधाभास वरिष्ठ पत्रकार और लेखिका मृणाल पाण्डे के सामने तब आए जब वे भारतीय स्त्री के स्वास्थ्य के बारे में जानने के लिए अपनी यात्रा पर निकलीं। अपने सफ़र में जल्द ही उन्हें मालूम हो गया कि यह सिर्फ़ भारत की सार्वजनिक स्वास्थ्य प्रणाली और प्रजनन स्वास्थ्य के इतिहास का दस्तावेज़ीकरण भर नहीं है, बल्कि एक व्यापक यथार्थ का सामना करना है। स्वयंसेवी कार्यकर्ताओं और स्वास्थ्यकर्मियों से बातचीत करने के लिए आर्इं महिलाओं से उनके अपने जीवन और शरीर के बारे में सुनकर उन्हें कुछ बड़ी वास्तविकताओं का बोध हुआ।
नतीजा है स्त्रियों के जीवन के विवरणों से रची हुई यह कृति, जो हमें बताती है कि स्त्रियाँ अपने वातावरण से कैसे प्रभावित होती हैं, औरत और मर्द की सेक्सुअलिटी को लेकर उनकी धारणाएँ क्या हैं; इसके अलावा गर्भधारण के रहस्य, जन्म देने के सुख, बाँझपन के भय, ग़ैरक़ानूनी गर्भपात और किशोरियों की सूनी दुनिया—इन सबके ब्यौरों से यह पुस्तक बुनी गई है।
मृणाल पाण्डे ने इस पुस्तक में जनसंख्या नीति और जनकल्याण में राज्य की भूमिका जैसे मुद्दों पर भी विमर्श किया है। और इस सबके बीच वे उन स्वयंसेवी संगठनों में अपना गहन विश्वास भी व्यक्त करती हैं, जिनकी कोशिशों के चलते स्त्रियाँ अपने जीवन की अँधेरी गलियों और ख़ामोशियों से बाहर आ रही हैं।
